Alliant | GMCF is committed to safer, more effective care for Medicare beneficiaries, as a result of its quality improvement work with Georgia health care providers. We engage providers at all levels of performance in rapid-cycle projects for collaborative learning and action that accelerate health care quality improvement.
EHR
Alliant | GMCF strives to improve participation in the physician quality reporting
system and increase the use of EHR for care management by:
- Ensuring that practices have integrated Health Information Technology and data exchange
infrastructures in their practices by demonstrated use of decision support tools
to improve quality of care, improved care coordination and better engagement of
patients and families
- Helping physicians take advantage of streamlined practices in the area of workflows,
data reports and identification of registry functions for provider clinical information
exchange to improve quality and efficiency of patient care
- Engaging physician practices with certified EHRs to begin participating in PQRS
using EHR-based reporting
PQRS
Physician Quality Reporting System:
PQRS is a quality reporting initiative implemented by CMS. The Georgia QIO will
engage physician offices with certified EHRs to begin participating in PQRS using
EHR-based reporting. Alliant | GMCF will assist the offices in data submission for
PQRS using EHR-based reporting.
Cardiovascular Health
Quality improvement specialists will work to ensure that health information technology
is used to improve cardiovascular health through care coordination, monitoring,
patient engagement and spreading of best practices.
Immunizations & Screenings
- Improving flu immunizations of patients ages 50 and older during the flu season
- Improving pneumococcal immunization of patients ages 65 and older
- Improving appropriate low-dose aspirin therapy use in patients with ischemic vascular
disease
- Improving BP control in patients with hypertension
- Improving LDL-C control among adults with ischemic vascular disease
- Improving tobacco cessation intervention among adult patients who smoke (screening
and cessation counseling)
- Improving colorectal cancer screening in patients ages 50-75
- Improving breast cancer screening in women ages 40-69
- Identifying and improving disparities within identified communities within the state
