Chronic Kidney Disease

Chronic Kidney Disease (CKD) is the ninth leading cause of death in Georgia.

The mortality rate of CKD in Georgia is 30-40% higher than the national average since 1980. Eighteen percent of all African-Americans over 65 have kidney disease and one third of them don’t even know it.

The CKD Quality Initiative assists physicians, hospitals, community health centers, and other providers with detecting the incidence and slowing the progression of CKD among Georgia’s Medicare beneficiaries. Through free education and technical assistance, access to patient education and outreach materials, and networking with other providers who care for CKD patients in their community, physicians have the opportunity to improve the quality of care of those who are disproportionately affected due to co-morbidities, race, gender or socioeconomic status.

The CKD Quality Initiative also works with renal organizations, state government agencies, provider groups and patients to develop, implement and sustain changes on a system level.
