Diabetes Education Program
Living Well with Diabetes…
Your Body. Your Health. Your Life.
The best way to take care of your body – especially if you have diabetes – is to be aware of the latest information relating to your condition. One way to achieve that is to attend a Diabetes Self-Management Education (DSME) program. You will receive the information you need to manage your diabetes, with topics such as meal planning, physical activity, warning signs and more.
Not treating and managing your diabetes could lead to many complications, such as blindness, amputations, dialysis, or even early death. The difference between living a fully productive, healthy life and one that is needlessly shortened and difficult – is in your hands.
More than 20 million Americans have diabetes, and African-Americans are 1.6 times more likely to be diagnosed with diabetes than Caucasians. In Georgia, diabetes is the seventh leading cause of death. It is estimated that 500,000 Georgians have been diagnosed with diabetes, while another 200,000 are unaware that they have the disease.
GMCF, the Medicare Quality Improvement Organization for Georgia, is working to support DSME efforts for patients with diabetes in the African-American and Hispanic populations. DSME is a proven intervention that allows patients to control their disease by working with their health care provider.
