GMCF understands that improving the health and well-being of people with diabetes is a main concern for you. More than 13 percent of all African-Americans have diabetes, and one-third of them don’t even know it. GMCF is implementing a Diabetes Self-Management Education (DSME) program in an effort to reduce disparities and adverse outcomes in minority, underserved, Medicare patients with diabetes.
You can help by giving patients the education and tools they need to control their blood glucose, blood pressure and lipids through preventive care.
These free classes will be offered to eligible African-American Medicare beneficiaries in the following counties: Bibb, Dougherty, Muscogee, Clayton, Fulton, Richmond, Cobb, Gwinnett, Spalding, DeKalb, Henry and Troup.
