Improving the health and well-being of your patients with diabetes is probably a main concern for you. For your African-American and Hispanic patients – who are at greater risk than the general population – it’s more important than ever. By working with GMCF and referring your patients to our no-cost Diabetes Self-Management Education (DSME) program, you’ll give them the tools they need to manage their diabetes.
GMCF is offering these free classes to eligible African-American and Hispanic Medicare beneficiaries in the following counties:
