Acute Care Hospitalization (ACH)

Improvement in the acute care hospitalization (ACH) quality of care measure is demonstrated by a decrease in the number of patients who had to be admitted to the hospital. Improved communications and patient monitoring might reduce the number of hospitalizations considered unnecessary. More information is available on the ACH page of the MedQIC Web site.
