Quality Stairsteps			Improvement Sustained for One Year
Milestones reached by Georgia home health agencies participating in the Home Quality Initiative		Statistically Significant Improvement	Engraved award presentation during a quality improvement event. A press release and photo will be sent to local media outlets once an agency remains for one year at, or above, the statistically significant improvement level acheived in Step 3. Note: Agencies must collect data for at least 12 months to qualify, and the 12-month data collection cycle varies by agency.

OBQI Training	Step up on Quality Staircase for completing a Plan of Action.
Framed certificate upon completion of training in Outcome Based Quality Improvement (OBQI).
