Congestive Heart Failure (CHF) Initiative

Working to improve functional status, decrease resource utilization and improve the mortality rates for patients hospitalized with heart failure in Georgia.

There are more than 740,000 admissions annually for Medicare patients in the United States with a primary diagnosis of heart failure. The prevalence of heart failure is rising dramatically with the aging of the U.S. population.
