Independent Care Waiver Program
The Independent Care Waiver Program allows for the provision of services to adult individuals with physical disabilities or traumatic brain injuries. Services are provided in the individual’s home or in the community as an alternative to nursing facility placement. The DMA-6 is the form used to determine level of care. The DMA-80 is the form utilized to request prior authorization of ICWP services for approved participants. Providers may submit the DMA-6 and DMA-80 via the Georgia Health Partnership (GHP) web portal.
To accommodate training needs, an ICWP Web Entry training session has been recorded and is available for viewing. To access the recorded session, you will be prompted to complete a registration page. (The registration information will be used to provide updates regarding future training sessions offered by GMCF.) Upon completion of the registration, access will be granted to the WebEx file and viewer. Instructions on how to download the WebEx file and viewer will be provided at that time.
