What is the Nursing Home Quality Initiative (NHQI)?

The Centers for Medicare & Medicaid Services (CMS) at the U.S. Department of Health and Human Services publicly reports quality measures for every Medicare and Medicaid certified nursing home. The quality measures for each nursing home are reported on NURSING HOME COMPARE at Medicare's Web site. GMCF helps consumers understand and use the measures as a tool for assessing nursing home facilities.

Advancing Excellence in America's Nursing Homes is a 2-year, coalition-based campaign to promote excellence in caregiving for nursing home residents. Click here for details.

Increasingly, there is evidence of serious deficiencies in quality of care for Medicare beneficiaries undergoing transitions across sites of care. In order to address this deficiency, GMCF, under the Care Transitions theme, will champion community care transition interventions to measurably improve post-acute care coordination and reduce rehospitalization rates in a targeted Georgia community.
