Our Initiative with Participating Practices

GMCF is looking to collaborate with practices like yours that have an existing ambulatory EHR product that is CCHIT certified. Selected practices will have the opportunity to work with GMCF to improve the delivery of preventive care services by maximizing EHR capabilities.

Participation in the initiative will prepare your practice for upcoming public reporting initiatives.

As a result of this initiative, you can:

• Increase mammography rates
  
• Increase CRC screening rates
  
• Improve influenza vaccination rates
  
• Improve pneumococcal vaccination rates
  
• Build your patient’s self-management skills
  
• Redesign your workflow to maximize staff time and efficiencies

GMCF has trained professionals on staff to provide you with:

• On-site consultations
  
• Customized education programs, tools, and resources
  
• Teambuilding and process improvement training
  
• Practice-specific assessments, improvement plans, implementation plans, workflow redesign and evaluation

Participating in this initiative is an opportunity to:

• Prepare your practice for upcoming public reporting initiatives by using the EHR to improve care management
  
• Increase your capability to improve patient care delivery by tracking each preventive service
  
• Educate your patients about the practice’s performance on publicly reported measures of quality care
  
• Collaborate with colleagues across care settings to ensure that Medicare patients take advantage of appropriate preventive services covered by Medicare
  
• Learn and adopt quality improvement methodologies and teambuilding strategies aimed at improving clinical outcome measures
